Provides direct nursing care including assessment, plan of care, education, and evaluation for patients with terminal illness who have elected the Hospice benefit. Performs patient duties necessary to maintain flow of patient care and coordinates delivery of care by other team members. Supports patient, family, and significant others and responds to their needs after business hours, holidays, and weekends.
