Waynesboro Family Medical Associates is a busy, friendly, independent medical office that has been established in Waynesboro for over 40 years. We are now looking for an additional RN Care Coordinator to join our team of two RNs. We provide care to all ages from infant to the elderly and need an upbeat, energetic professional with a strong service ethic and enthusiastic desire to make a difference in the lives of our patients. We are a family oriented office that truly values our employees and their concerns.

As a Nurse with recent clinical experience, you will be providing Chronic Care Coordination support to our patients under the direction of our 2 Physicians and 3 Nurse Practitioners. The Chronic Care Coordinator works in collaboration with patient, family, caregiver and medical provider to assist the patient in the management of their chronic conditions between office visits. These services are provided by telephone contact and the occasional office visit. The Chronic Care Coordinator provides health coaching, education, referral services, and coordination with the medical providers to assure that the patient needs are met and that condition changes are addressed immediately. They coordinate transition of patients returning to their residence after a hospitalization or skilled facility stay. They also recognize patients with chronic care conditions and are able to coordinate their admission to our Chronic Care Management program with timely documentation.

Qualifications:

RN with a current active license in the state of PA, Home Health experience preferred
Minimum of 2-3 years work experience in a healthcare setting involving patients with
complex chronic conditions preferred
Strong interpersonal and communication skills to ensure excellent customer service
Organized and the ability to multi-task with time management skills
Ability to recognize a problem and recommend solutions
Knowledge of the local and state community services or willingness to learn
Ability to communicate effectively in person and using technology
Basic computer skills including previous work experience with an electronic medical
record
Ability to spend 80% of the day sitting in front of a computer and using the telephone
