\n\nQualifications:\n

• 2 years of case management experience required.
\n
• LCSW license required.
\n
• Oregon RN license required if candidate is a nurse.
\n

\n\nPreferred Experience:\n

\n
• Experience in teaching hospital settings.
\n
• Experience in community hospital settings.
\n
• LTAC (Long-Term Acute Care) experience.
\n
• Trauma Level I and II experience.
\n

\n\nSkills Required:\n

• Case Management/Utilization Review
\n
• Care Coordination
\n
• Discharge Planning
\n
• Needs Assessment/Ordering DME
\n
• Advance Directives
\n

\n\nKnowledge Areas:\n

\n
• Protective Services (Adult and Child)
\n
• Emergency Detention
\n
• Guardianship and Power of Attorney
\n
• Health and Community Service Programs (Medicare, Medicaid, etc.)

• Major Disease Processes (e.g., Cancer, Diabetes, Dementia)

\n\nResponsibilities:\n

• Conduct assessments and develop plans of care to meet patient and caregiver needs.
\n
• Facilitate social and psychological well-being of residents in nursing homes.
\n
• Manage discharge planning and facilitate transfers to physician appointments.
\n
• Document care delivery, plans, and patient responses effectively.
\n
• Educate patients and caregivers to facilitate successful discharge processes.
\n
