Traveler Flight Form

Please arrive in the city within 48 hours of your first shift. Luggage is not included with your travel; you must anticipate paying. Arrive 2 hours early to the airport. If you miss your flight, or the flight is canceled please notify us immediately, any changes in flight cost will be directly billed to traveler If you do not show up or decide not to go after your flight has been booked you will be charged for the entire flight and travel reimbursement, plus $250 administrative fee.
